FROM WISDOM TOOTH TO AIRWAY EMERGENCY: DEEP NECK SPACE INFECTION ARISING FROM IMPACTED THIRD MOLARS

Brief Summary of the Grand Round
Deep Neck Space Infection (DNSI) remains one of the most potentially life-threatening complications of odontogenic infections.

Impacted mandibular third molars (wisdom teeth) can serve as a nidus for infection, leading to rapid spread through the fascial planes of the neck.

If not recognized and managed promptly, these infections may result in airway compromise, mediastinitis, sepsis, and death.

Odontogenic infections account for a significant proportion of deep neck space
infections seen in maxillofacial and emergency practice.

Early diagnosis, prompt airway assessment, appropriate antimicrobial therapy, and timely surgical intervention are critical determinants of patient outcomes.

This session will explore the anatomical pathways of spread, clinical presentation, diagnostic approach, multidisciplinary management, and preventive strategies related to deep neck space infections arising from impacted third molars.

Where This Course Fits In
This session forms part of our ongoing clinical education, patient safety, emergency preparedness, and quality improvement initiatives.
It aims to enhance clinicians’ ability to identify early warning signs of severe odontogenic infections, institute timely interventions, prevent airway emergencies, and improve multidisciplinary collaboration in patient management.

Learning Objectives:
At the end of this session, participants should be able to:

  • Define Deep Neck Space Infection (DNSI) and its clinical significance.
Describe the anatomy of the deep cervical fascial spaces and pathways of
    odontogenic spread.
  • Identify impacted third molars as a potential source of severe neck infections.
Explain the microbiology and pathophysiology of odontogenic deep neck
    space infections.
  • Recognize the clinical features and red flags suggestive of impending airway
    compromise.
  • Differentiate between localized odontogenic infections and deep neck space
    involvement.
  • Discuss the role of imaging modalities in diagnosis and treatment planning.
Outline evidence-based principles of airway management in patients with
    DNSI.
  • Review surgical and medical management options, including incision and
drainage, antimicrobial therapy, and multidisciplinary care.
Highlight preventive measures and indications for prophylactic removal of
    impacted third molars.
