Ruptured Uterus – A Prevailing Obstetric Mishap!
The April 2025 Grand Round is based on case presentations of 2 young women (in their thirties) with significant clinical features as follows:
- A booked patient with a history of previous myomectomy and in-vitro fertilisation (IVF) twin conception who experienced a spontaneous uterine rupture at 18 weeks’ gestation associated with hypovolemic shock.
- An unbooked patient referred to the Duchess International Hospital following a history of a missed miscarriage at approximately 20 weeks’ gestation and subsequent use of misoprostol, leading to an unfortunate uterine rupture and hypovolemic shock.

Where does this Course fit in?
Pregnancy is a high-risk period for a mother. The increased frequency of uterine surgical procedures (myomectomy, hysterectomy, Caesarean section, etc.) has further enhanced the risk of a uterine rupture, both in labour and spontaneously during pregnancy.
The dearth of skilled obstetricians and experienced midwives in many communities, and the likelihood of unsupervised labour interventions in those circumstances also significantly increases the possibility of a uterine rupture occurring.
This severe, and not uncommon complication is associated with significant morbidity and mortality in women of childbearing age, often with serious medical and reproductive outcomes such as haemorrhage, blood transfusion reactions (from multiple transfusions), acute kidney injury, hypoxic encephalopathy, peri-partum hysterectomy, Sheehan’s syndrome, amenorrhea, and subsequent female infertility.
