Cough is one of the commonest reasons for encounter in the family medicine clinic. It is a symptom rather than a disease. However, when cough becomes chronic and associated with systemic symptoms like fever, weight loss, fatigue, excessive sweat, it leads to concern about serious underlying medical conditions.
Tuberculosis is endemic in Nigeria, with the country having the highest burden in Africa and sixth in the world. This has thus made pulmonary tuberculosis the first and most common differential of chronic cough with systemic symptoms. Evidence has however shown that there are occasional misdiagnosis and sometimes unnecessary empiric treatment for tuberculosis in diabetes patients.
